Six-month-old Iris Day died on 2 December 2016 after going to Colchester Hospital while in respiratory distress. There were signs Iris was deteriorating which were not identified by the treating clinicians. Giving a narrative determination the Coroner found that there were failings in the care provided by the Hospital.
